East Grinstead Hockey Club requires completed Colts and Fillies Membership forms for all Colts and Fillies (boys and girls) under the age of 18(School Years 8, 9, 10, 11, 12 and 13). All details will be kept in a secure database with access restricted to authorised club officers only.
It is important to ensure that the email addresses and mobile numbers given below are those which are regularly checked as these are our primary means of communicating with club members.
As with all volunteer amateur Sports Clubs, we rely heavily on the support of local sponsors to help us operate the club and to invest strategically in infrastructure and services. As a result, we will, from time to time, include sponsor information in our Club Newsletters which are emailed to the membership. All Club Members are automatically subscribed to these newsletters to enable us to circulate important communications information.
SECTION 1: Colt / Filly Member Information
EGHC requires information relating to the Colt or Filly Member's health in order to ensure correct duty of care when under the supervision of Club Officials and Volunteers. In addition we request certain other pieces of information.
In cases where parents do not consent to first aid being administered, the parent will be expected to be by the pitch during all sessions and matches.
EGHC is building close working relationships with a number of schools in the area. By sharing the number of EGHC members attending each school, we can continue to strengthen these relationships.
Whilst it is not compulsory for information related to ethnicity to be recorded, we explain at the foot of this form that reasons why this information is considered important.
SECTION 2: Parent/Guardian Information

SECTION 7: Supplementary Information
Ethnicity & Disability
Sport can and does play a major role in promoting the inclusion of all groups in society. However, inequalities have existed within sport particularly in relation to gender, race and disability. Sport England and England Hockey are committed to promoting and developing sports equity, which is about fairness in sport, equality of access, recognising inequalities and taking steps to address them. By monitoring the profile of people in sports clubs, national governing bodies of sport and Sport England can identify any issues relating to under representation of different groups and can develop strategies to ensure that all people have the opportunity in the future to develop and progress in sport. England Hockey requests this data from clubs as part of the annual affiliation process and completing this data accurately enables the club to give an accurate picture to England Hockey on our membership.
